Country to Coast Queensland (CCQ) delivers the Australian Government's Primary Health Network (PHN) for Central Queensland, Wide Bay and Sunshine Coast. As a not-for-profit organisation, we coordinate primary and preventive health care across 161,000 km² and a population of more than 820,000 people, from Glass House Mountains to north of Yeppoon and west to Emerald. We are one of 31 PHNs funded by the Australian Government.What we do:• Coordinate primary health services to meet local needs• Improve the patient journey between primary and acute care• Support general practice and the broader primary care workforce• Improve access to services in rural and remote communities• Reduce pressure on hospitals by strengthening community-based careOur events bring together health professionals, community leaders and service providers to share insights, build connections and support better health outcomes across our region.
